All applications MUST be submitted online. No paper copy applications will be accepted. Only complete applications submitted electronically will be reviewed for acceptance. Please ensure you have all documents ready to submit prior to filling out an application. Any missing documents will make your application incomplete.

To apply to LSCO’s Associate Degree Nursing Program:

1. Apply to be an LSCO student;
2. Complete the application below after acceptance to LSCO;
3. Upload required documents - unofficial academic transcripts, HESI A2 scores, and proof of active Certification of Nurse Aid; proof of active Certification of PCT; OR VNSG 1323/VNSG 1160; OR NURA 1301/NURA 1160; OR NURA 1001/NURA 106
