  • Official transcripts showing completion of required education and training from LSCO
  • Proof of passing the TDLR Massage Therapist Jurisprudence Exam
  • Proof of passing the MBLEx Exam (within time limitations)
    • Set up account
    • Schedule exam
    • Fee - $265
    • Complete the exam at a Person VUE Testing Center
  • The non-refundable application fee is $100.
